探讨肩关节撞击综合征诊断中磁共振成像的应用效果及检出率观察

摘要   【摘 要】目的:探讨并分析磁共振成像应用在肩关节撞击综合征的临床诊断中的应用效果。方法:于 2016年 7月到 2018年 8月在本院选取肩关节撞击综合征患者 50例作为观察组研究对象,于 2016年 7月到 2018年 8月在本院进行体检的健康患者 50例作为对照组研究对象。所有患者均接受磁共振成像检查,观察两组患者的磁共振成像结果,比较两组患者的 AHD值和肩峰形态。结果:观察组 50例肩关节撞击综合征患者中,均出现冈上肌腱形态变化和冈上肌腱信号变化的情况,其中完全撕裂的患者有 7例,部分撕裂的患者有 21例,还有 21例患者的临床特征不明显 ;同时,存在肩峰下滑囊增厚的患者有 14例,存在肩峰下滑囊积液的患者有 21例,存在关节盂唇撕裂的患者有 7例,未发现存在冈上肌萎缩和三角肌萎缩的患者。观察组肩关节撞击综合征患者的 AHD值为( 5.19±0.52) mm,对照组患者的 AHD值为( 8.74±0.64) mm,两组相比, P<0.05。观察组肩关节撞击综合征患者的肩峰形态和对照组患者的肩峰形态相比,无明显差异, P>0.05。结论:在肩关节撞击综合征的临床诊断中应用磁共振成像,可清楚显示患者的损伤程度、肌腱病变等情况,为肩关节撞击综合征的临床诊断提供更为准确的判断依据。    【关键词】肩关节撞击综合征 ;磁共振成像 ;应用效果  [Abstract] Objective: To explore and analyze the application of MRI in the clinical diagnosis of shoulder impact syndrome. Methods: from July 2016 to August 2018, 50 patients with shoulder impact syndrome were selected in our hospital as the study object of the observation group, and 50 healthy patients who had physical examination in our hospital from July 2016 to August 2018 as the control group. All patients were examined by MRI. The results of MRI in the two groups were observed. Results: in the observation group, there were changes of supraspinatus tendon shape and supraspinatus tendon signal in 50 patients with shoulder impingement syndrome, including 7 patients with complete tear, 21 patients with partial tear and 21 patients with no obvious clinical features; At the same time, there were 14 patients with the thickening of the acromiocyst, 21 patients with the effusion of the acromiocyst, 7 patients with the tear of the glenoid lip, and no patients with the atrophy of the supraspinatus and deltoid muscles. The ahd value of the patients in the observation group and the control group was (5.19 ± 0.52) mm and (8.74 ± 0.64) mm respectively, P < 0.05. There was no significant difference between the observation group and the control group (P > 0.05). Conclusion: magnetic resonance imaging can clearly show the degree of injury, tendon lesions and other conditions in the clinical diagnosis of shoulder impact syndrome, and provide a more accurate basis for the clinical diagnosis of shoulder impact syndrome.
