简介:AbstractPurpose:Fingertip injuries are common in industrial production activities as well as in domestic work. Loss of pulp hampers daily life activities. Functional and aesthetic aspects are important in fingertip reconstruction. The bone is usually exposed along with soft tissue loss. Therefore to reconstruct the pulp flap with adequate bulk is required.Methods:We reported a case series of 12 patients with the injury over the volar aspect of distal phalanx of the index or middle finger. In all cases, laterally based thenar flap was chosen. The flap donor site was closed primarily in most of cases, while 4 patients required skin graft. The flap was detached between 2-3 weeks. Functional assessment was done using static and dynamic 2-point discrimination and range of motion at each joint. The aesthetic outcome was assessed through questionnaire. The results were analyzed using the unpaired t-test (SPSS version 21).Results:Partial necrosis occurred in 2 cases while rest of flaps survived successfully. Static 2-point discrimination ranged from 6-10 mm, mean 8.6 mm; and dynamic 2-point discrimination ranged from 8-10 mm, mean 8.9 mm. The mean satisfaction score was (4.0 ± 0.55).Conclusion:Thenar flap is a good choice for reconstruction of the finger pulp as it provides the bulk with good functional and aesthetic outcome.
简介:AbstractIntroduction:Subungual exostosis (SE) is a relatively uncommon benign bone tumor that occurs in the distal phalanges of the toes or fingers.Case presentation:An 8-year-old girl presented for treatment of an acquired mass on the distal right index finger. The patient was preoperatively diagnosed with a benign bone tumor. Surgical resection of the distal right index finger mass was performed under general anesthesia, and histological examination of the resected tissue supported a diagnosis of SE of the right index finger. The surgical outcome was good, with no surgical site infection. Throughout 15 months of follow-up, the patient was asymptomatic with no recurrence.Conclusion:Surgical resection of a mass on the distal right index finger resulted in absence of recurrence during 15 months of follow-up. SE is a benign and uncommon lesion that is infrequently encountered by physicians. This infrequent occurrence may result in delays in diagnosis and treatment. Complete excision of the lesion and careful separation from underlying nail bed structures results in total resolution of the lesion, while providing the lowest risk of recurrence.

简介:Thefingerjointlinesdefinedasfingercreasesanditsdistributioncanidentifyaperson.Inthispaper,weproposeanewfingercreasepatternrecognitionmethodbasedonLegendremomentsandprincipalcomponentanalysis(PCA).Afterobtainingtheregionofinterest(ROI)foreachfingerimageinthepreprocessingstage,LegendremomentsunderRadontransformareappliedtoconstructamomentfeaturematrixfromtheROI,whichgreatlydecreasesthedimensionalityofROIandcanrepresentprincipalcomponentsofthefingercreasesquitewell.Then,anapproachtofingercreasepatternrecognitionisdesignedbasedonKarhunen-Loeve(K-L)transform.ThemethodappliesPCAtoamomentfeaturematrixratherthantheoriginalimagematrixtoachievethefeaturevector.Theproposedmethodhasbeentestedonadatabaseof824imagesfrom103individualsusingthenearestneighborclassifier.Theaccuracyupto98.584%hasbeenobtainedwhenusing4samplesperclassfortraining.Theexperimentalresultsdemonstratethatourproposedapproachisfeasibleandeffectiveinbiometrics.

简介:Objective:Toobservetherevascularizationandtheopportunityofcross-fingerflap.Methods:Ananimalmodelwasdevelopedtopermitdailymonitoringofneovascularizationoftheflapwithautoradiography,tissuetransparenttechnique,grossobservationandhistologicalexamination.Results:Therevascularizationoftheflapwaschieflyraisedfromthesurroundingtissues.Thepediclesof334cross-fingerflapsofthepatientsweresuccessfullydividedfrom1to5dafteroperations,averagely3.3d.Allofthecasesshowedsatisfatoryresultsaccordingtoafollow-upsurveyof3to72mon.theresultsfurtherprovedthattherevascularizationofthecross-fingerflapshadaccomplishedwithin3d.Conclusions:Itsuggeststhatthedivisionofacrossfingerflapcanbecarriedoutbetweenthe3rdand5thdayafteroperationwithahighdegreeofsafety.
简介:AbstractIntroduction:Bowen’s disease is a squamous cell carcinoma in situ. It is more common on the head and neck, less common on the nail units in which it usually involves the second to fourth fingers of both hands. Here, we report a case of Bowen’s disease involving the left fifth finger of a 78-year-old Chinese woman.Case presentation:The patient was treated with nine sessions of topical 5-aminolevulinic acid-photodynamic therapy (5-ALA-PDT), with a one-week interval between sessions. The 5-ALA cream was applied to the lesion and the region surrounding its marginal diameter by 0.5 cm. After 3-4 hours, the lesion was irradiated with 635 ± 10 nm red light at an intensity of 80 mw/cm2 for 20 minutes. An excellent result with a topical 5-ALA-PDT was reached.Discussion:PDT involves the application of a combination of a photosensitizing drug and light to cause selective damage to the target tissue. PDT is more appropriate than surgical excision for lesions located in sites with poor healing ability.Conclusion:PDT appears to be effective, safe and has a better cosmetic outcome in the treatment of Bowen’s disease of the finger unit.

简介:Thephotoplethysmogram(PPG)ofapulsewave,similarinappearancetothearterialbloodpressure(ABP)waveform,containsrichinformationaboutthecardiovascularsystem.ThedecaytimeconstantRC,equaltotheproductofperipheralresistanceRandtotalarterialcomplianceC,isameaningfulcardiovascularmodelparameterinvascularassessment.UsingoramelioratingtheexistingABPmethodsdoesnotachieveasatisfactoryestimationofRCfromthePPGvolumepulse(VRC).Thus,anovelnon-iterativeshapemethod(NSM)ofevaluatingVRCisintroducedinthispaper.Themathematicexpressionbetweenanovel,readilyavailablemorphologicalparametercalledtheareadifferenceratio(ADR)andVRCwasestablished.AsitwasdifficulttocalculateVRCfromthecomplicatedexpressionanalytically,werecommendestimatingitusingapiecewiselinearinterpolationcriterion.Also,sincetheeffectofthePPGmagnitudeiseliminatedinthecalculationofADR,precaliberationornormalizationisdispensableintheNSM.ResultsofhumanexperimentsindicatedthattheNSMwascomputationallyefficient,andthesimulationexperimentsconfirmedthattheNSMwastheoreticallyavailableforABP.
