Thecombinationofradiotherapy(RT)andfunction-preservingsurgeryisthemostusualcontemporaryapproachinthemanagementofsofttissuesarcomas(STS).Pre-andpostoperativeRTresultinsimilarlocalcontrolrates,asshownbyalandmarktrialinextremitySTS.Inthisreview,theroleofRTinthemanagementofextremitySTSwillbediscussed,butSTSinothersites,includingretroperitonealSTS,willalsobeaddressed.ThefocuswillconsidervariousaspectsofRTincludingstrategiestoreducethevolumeoftissuebeingirradiated,dose,scheduling,andthepossibleofomissionofRTinselectedcases.Finally,technologyadvancesthroughtheuseofintensity-modulatedradiotherapy(IMRT),image-guidedIMRT,intraoperativeradiotherapy(IORT)andparticletherapywillalsobediscussed.
